Accounts and procedures

If the account has been disconnected for more than a month:

  • Photocopy of valid ID of owner
  • Affidavit of reconnection (for non-appearance of the registered owner)
  • Authorization letter and photocopy of ID of representative (for non-appearance of the registered owner)
  • Payment of necessary bills/fees
    • unpaid water bill
    • latest water consumption from reading date to date of disconnection
    • reconnection fee
    • remaining balance in the installation fee, materials, and other fees if any

 If the account has been disconnected within a month:

  • Payment of necessary bills/fees
    • unpaid water bill
    • latest water consumption from reading date to date of disconnection
    • reconnection fee
    • remaining balance in the installation fee, materials, and other fees if any

Note: An inspection of the water line will be required for accounts disconnected on or before 2013

Requirements for New Water Service Connection

  • Accomplished Application Form
  • 1pc. (2×2) picture
  • Photocopy of Community Tax Certificate (Cedula)
  • Photocopy of 1 any government issued ID, submit 2 if no Community Tax Certificate (Cedula)
  • Photocopy of Absolute Deed of Sale / Land Title (optional)
  • Photocopy of Lease Contract (for Business and UPLB staff housing)
  • Personal appearance for the signing of Memorandum of Agreement (MOA)
  • Special Power of Attorney and Photocopy of ID of representative, in case of non-appearance of applicant
  • Payment of necessary bills/fees

  If account is active:
  • Affidavit of Transfer of Name
  • Authorization/waiver letter from the previous owner
  • Photocopy of ID of the previous owner
  • Photocopy of Deed of Absolute Sale/Land Title or P500.00 water bill deposit
  • Accomplished Application Form
  • Personal appearance for the signing of Memorandum of Agreement (MOA)
  • Special Power of Attorney and Photocopy of ID of representative, in case of non-appearance of applicant

 

If account is disconnected:

  • Affidavit of Reconnection and Transfer of Name
  • Authorization/waiver letter from the previous owner or Photocopy of Death Certificate (if the previous owner is deceased)
  • Photocopy of ID of the previous owner (if still living)
  • Photocopy of Deed of Absolute Sale/Land Title or P500.00 water bill deposit
  • Accomplished Application Form
  • Payment of necessary bills/fees
  • Personal appearance for the signing of Memorandum of Agreement (MOA)
  • Special Power of Attorney and Photocopy of ID of representative, in case of non-appearance of applicant

Qualifications

  • Individuals with ages 60 years old and above
  • Water bill account is registered to LARC for at least one year under the name of the senior citizen
  • Water consumption does not exceed 30 cubic meter

Requirements

  • One old LARC water bill receipt
  • Barangay certificate of residency
  • Senior Citizen ID
  • Senior Citizen’s authorization letter and representative’s ID, in case of senior citizen’s non-appearance
  • Accomplished Application form
